If a fibroid needs treatment, the next question is how. Dr. Sanjana L, gynaecologist and keyhole surgeon in HSR Layout & Attibele, compares medical management, laparoscopy and open surgery.
First, Does It Even Need Treating?
Before comparing treatments, the honest first question is whether a fibroid needs treatment at all — many do not. If you have not already, our guide on fibroid symptoms and when surgery is needed is a good starting point. This article assumes treatment has been recommended, and looks at how the options compare.
Option 1: Medical Management
For fibroids whose main problem is heavy bleeding, medication often comes first:
- Hormonal treatments and a hormonal IUD can reduce bleeding
- Other medicines can temporarily shrink fibroids or control symptoms
- Iron corrects anaemia from blood loss
Medical management suits women who want to avoid surgery, or as a step before it. It controls symptoms but does not remove the fibroid, so symptoms can return.
Option 2: Laparoscopic (Keyhole) Surgery
A laparoscopic myomectomy removes fibroids through small incisions using a camera and fine instruments. Where suitable, it is usually the preferred surgical option because it offers:
- Smaller incisions and minimal scarring
- Less pain and faster recovery (often days rather than weeks)
- Preservation of the uterus, protecting future fertility
Hysteroscopic surgery (through the cervix, no external incision) is ideal for fibroids inside the uterine cavity.
Option 3: Open Surgery
Open (abdominal) myomectomy or, where appropriate, hysterectomy is reserved for situations where keyhole surgery is not the safest choice — for example very large or numerous fibroids. Recovery is longer, but for certain cases it is the right, safe option.
How the Decision Is Made
The best option depends on the size, number and position of the fibroids, your symptoms, and — importantly — whether you want to preserve fertility. A fellowship-trained (FMAS) surgeon like Dr. Sanjana favours the least invasive option that safely solves the problem. You can read more on our uterine fibroids treatment page.
Frequently Asked Questions
Can fibroids be treated without surgery?
Yes, when the main problem is bleeding. Medical and hormonal treatments can control symptoms, though they do not remove the fibroid, so symptoms may return.
Is laparoscopic fibroid surgery better than open surgery?
For suitable cases, keyhole surgery offers less pain, faster recovery and smaller scars while preserving the uterus. Very large or numerous fibroids may still need open surgery.
Will I be able to have children after fibroid surgery?
A myomectomy is specifically designed to remove fibroids while preserving the uterus, which supports future fertility. Your surgeon will discuss your individual situation.
How long is recovery after keyhole fibroid surgery?
Many women return to light activity within a few days and normal activity within one to two weeks, faster than open surgery.
