Ovarian Cysts: Which Ones Are Harmless and Which Need Treatment

Most Ovarian Cysts Are Nothing to Fear

An ovarian cyst is a fluid-filled sac on or inside the ovary. The word "cyst" sounds alarming, but the large majority are functional cysts — a normal by-product of the menstrual cycle that forms and then disappears on its own within a few weeks.

So when a scan reports a cyst, the real task is telling a harmless one apart from the small number that need attention. The Cysts That Are Usually Harmless

Functional cysts — follicular and corpus luteum cysts — form as part of ovulation. They:

  • Are very common in women of reproductive age
  • Usually cause no symptoms
  • Typically resolve on their own within one to three menstrual cycles
  • Need only a repeat scan to confirm they have gone

For these, the right response is reassurance and a short interval scan, not surgery.

The Cysts That Need a Closer Look

A cyst deserves more attention when it is:

  • Large, or growing on repeat scans
  • Persistent — still there after a few cycles
  • Causing pain, pressure or bloating
  • Showing complex features on ultrasound (solid areas, septations)
  • Associated with conditions such as endometriosis (endometriomas) or, rarely, other growths

One situation is urgent: sudden, severe, one-sided pelvic pain can signal ovarian torsion (a twisted cyst) — this needs immediate medical care.

How Cysts Are Assessed

Assessment is straightforward: a symptom review, a pelvic examination, and an ultrasound to characterise the cyst. Where the scan warrants it, blood tests or tumour markers add information. Together these usually make it clear whether a cyst can simply be watched or needs treatment.

When Treatment Is Needed

When a cyst is large, persistent, twisted or symptomatic, treatment is usually a laparoscopic (keyhole) cystectomy that removes the cyst while preserving healthy ovarian tissue — protecting your future fertility. Cysts, PCOS and Fertility

Having "cysts" on a scan is not the same as having PCOS. In PCOS, the ovaries show many small follicles rather than a single significant cyst, and the condition is hormonal and metabolic. Most simple ovarian cysts do not affect fertility, though conditions like endometriomas can — which is another reason accurate assessment matters.
