A normal ultrasound does not rule out endometriosis. Scans reliably show larger ovarian cysts of endometriosis, but superficial deposits on the pelvic lining and the adhesions that cause much of the pain are usually invisible on imaging. Diagnosis therefore rests on your symptom pattern, examination and, where needed, laparoscopy.

Pain that starts before bleeding and lasts through it, pain during or after intercourse, painful bowel movements or bladder discomfort that follows the cycle, pelvic ache on ordinary days, unusual fatigue, and difficulty conceiving. Period pain that stops you working or that painkillers no longer touch also deserves proper assessment.

Endometriosis is managed long term rather than cured, and it can recur after treatment including after surgery. That is not a reason to avoid treatment.
