Overview
Two opposite mistakes are made with fibroids, and both are common. The first is treating a fibroid because it exists — operating on a small, symptomless fibroid found incidentally on a scan, when watchful waiting would have been correct. The second is dismissing years of heavy bleeding as ordinary periods until a woman is significantly anaemic and exhausted. The size on the report matters far less than what the fibroid is doing to you.
Fibroids are benign muscular growths in the uterine wall, and they are extremely common — a large proportion of women have them by their forties, most without ever knowing. They matter when they cause heavy or prolonged periods, pressure on the bladder causing frequent urination, a feeling of fullness or a visibly distended abdomen, pain during intercourse, back or leg discomfort, or difficulty conceiving and recurrent miscarriage when they distort the uterine cavity. Position matters more than size: a small submucosal fibroid inside the cavity can cause far more bleeding and fertility trouble than a large one sitting on the outer wall.

No. Most fibroids are small, symptomless and found incidentally, and the right management is monitoring with a review scan. Treatment is driven by symptoms — heavy bleeding, anaemia, pressure effects, pain, or fertility problems — and by position, not by the existence of a fibroid on a report.
Yes. A myomectomy removes the fibroids and preserves the uterus, and can be done hysteroscopically for fibroids inside the cavity or laparoscopically for those in or on the wall. Hysterectomy is one option among several, not the default, and is generally reserved for women who have completed their family and have severe symptoms.
Most do not. Those that distort the uterine cavity — particularly submucosal fibroids — can reduce fertility and increase miscarriage risk, and removing them often helps. Fibroids on the outer wall usually have little effect on conception. Position is what decides this, which is why the scan needs to be mapped rather than just counted.
