Overview
Being told after a scan that you have fibroids is unsettling, and for most women the next thought is that a hysterectomy is coming. For the majority, it is not. Fibroids are non-cancerous growths in the muscular wall of the uterus, they are extremely common, and a great many are picked up incidentally on a scan done for something else and never cause a single symptom. Where there are no symptoms and the fibroid is not in a troublesome position, the correct answer is usually monitoring with a repeat scan, not an operation. Dr. Sanjana L sees women with newly diagnosed fibroids at Health Nest in HSR Layout Sector 2, and the visit generally starts with translating your scan report into plain language.
Position matters more than size. A small submucosal fibroid pushing into the cavity of the uterus can cause heavy bleeding and interfere with conception, while a considerably larger subserosal one sitting on the outer surface may cause nothing at all; intramural fibroids grow within the wall and behave somewhere between the two. Treatment follows a ladder — watchful monitoring, then medical management for heavy bleeding and the anaemia that follows it, then uterus-preserving surgery such as hysteroscopic or laparoscopic myomectomy when symptoms justify it. Dr. Sanjana is FMAS-trained in minimally invasive surgery, so keyhole options are weighed properly. Hysterectomy sits at the end of that ladder, not the start.

No. Many fibroids are found by chance, cause no symptoms and need nothing more than a repeat scan to confirm they are stable. Surgery is considered when there is heavy bleeding, anaemia, pressure on the bladder or bowel, pain, or an effect on fertility — and even then, uterus-preserving options come first.

Yes. A myomectomy removes the fibroids and leaves the uterus in place, performed hysteroscopically through the cervix for fibroids inside the cavity, or laparoscopically through small keyhole incisions for those in or on the wall. Dr. Sanjana is FMAS-trained in minimally invasive surgery, so these routes are assessed before hysterectomy is raised.
It depends almost entirely on position. Submucosal fibroids distorting the cavity of the uterus can interfere with implantation and are the ones most likely to need removal before conception. Fibroids sitting on the outer surface often have no effect at all. Many women with fibroids conceive and carry a pregnancy without difficulty.
